Maternity and neonatal care services are badly in need of an overhaul at national level to improve safety locally, the Health Services Safety Investigations Body (HSSIB) has warned in a review.
The national patient safety body’s “exploratory review” concluded that failures in maternity and neonatal safety are due to systemic issues at national level rather than isolated issues locally.1
The HSSIB team carried out 17 national stakeholder interviews and reviewed 35 safety concerns submitted to it, alongside one of its own reports published in 2021.2 These revealed several systemic issues that need to be tackled.
